Who Must Avail the Service:
Student who wishes to add, change & drop subjects after enrollment.
Schedule of Availability of Service:
Monday to Friday
7:00 a.m. – 6:00 p.m.
8:00 a.m. – 5:00 p.m.
Fees: Php 50.00 for reprinting of registration form
- Duly approved General Clearance
- Application for graduation
- Final rating form
Steps on How to Avail of the Service:
|Step 1. Proceed to the registrar’s office window 7 to secure the prescribed form||The personnel in-charge at window 5, checks the registration form of the student.|
|Step 2. Fill – up the form and proceed to the concerned officials for approval||
|Step 3. Proceed to the Cashier’s office & pay the required fees.||
|Step 4. Submit the duly accomplished form & receipt to the Registrar’s office encoding section||
